Job summary

The SNAP Nutritional Navigator role in Brooklyn provides direct, community-based assistance to help eligible individuals enroll in SNAP. You will lead outreach at distributions and events, guide clients through the complete SNAP application, and support recertifications with meticulous documentation.

You'll ensure accurate data entry into Salesforce, follow up to resolve barriers, and work under OTDA requirements to deliver end-to-end navigation and outcomes for low income households.

Qualifications

• Bachelor’s or Associate degree in social services or related field.
• 1–3 years experience in care coordination or case management.
• Familiar with Medicaid and Section 1115 Waiver.
• Comfort using cloud-based care-management systems (Unite Us, Findhelp).
• Strong trauma-informed communication and cross-cultural empathy.

This role focuses on delivering targeted outreach, conducting initial eligibility pre-screenings, and guiding clients step by step through the application and enrollment process. The Navigator ensures accurate document verification, supports clients through mandatory recertifications, and maintains consistent follow-up to resolve barriers to benefits. By offering compassionate, end to end navigation services, this position helps low income households successfully secure vital nutritional support.****Responsibilities****Community Outreach*** **Conduct on-site outreach** **at TCAH food distributions, community events, partner sites, and other strategic community locations.*** **Host informational sessions** **to educate the public about SNAP eligibility guidelines, benefits, and common misconceptions.*** **Proactively identify** **and engage potentially eligible households within the community to build a robust client pipeline.****SNAP Application Assistance*** **Guide clients** **step-by-step through the complete SNAP application process, ensuring a seamless and dignified experience.*** **Help clients gather****, organize, and verify required documentation prior to application submission.*** **Submit completed applications** **on behalf of clients and swiftly respond to any outstanding documentation requests from social services.****Recertification Support*** **Assist existing SNAP recipients** **with completing and submitting their mandatory periodic recertifications.*** **Identify potential bottlenecks** **in the recertification cycle to proactively prevent any interruptions or lapses in client benefits.****Client Follow-Up & Tracking*** **Follow clients closely** **through every stage of the state application timeline to ensure files move forward smoothly.*** **Track final outcomes** **of both initial applications and recertifications to ensure data integrity and measure program success.****Data, Documentation, & Compliance*** **Perform timely and accurate entry** **of all client services, interactions, and final enrollment outcomes into TCAH’s designated data systems, including Salesforce.*** **Maintain detailed documentation** **and files in strict compliance with federal, state, and grant-specific reporting requirements.****Performance Expectations & KPIs*** **The SNAP Nutritional Navigator is expected to actively contribute toward the program’s established Key Performance Indicators (KPIs). Performance will be evaluated based on the tracking and meeting of targets in the following areas:*** **Number of SNAP pre-screenings conducted*** **Total applications submitted and recertifications completed*** **Volume of unique households assisted*** **Frequency and reach of community outreach activities*** **Consistency of documented client follow-ups*** **Accuracy and timeliness of data entry and grant documentation****Supervision & Program Compliance****The Navigator operates under the direct supervision of the Senior Benefits Access Manager and works in close coordination with the broader Benefits Access leadership team.
