Job summary

Priority Dispatch Corp. seeks a SNAP Nutritional Navigator to provide community outreach, eligibility pre-screenings, and end-to-end assistance with SNAP applications and recertifications in New York. You will work under supervision of the Senior Benefits Access Manager, coordinating with the Benefits Access leadership team to meet OTDA program requirements and reporting standards.

Qualifications

• Education as specified; relevant field experience may be considered in lieu of degree.
• 1–3 years of experience in care coordination or human services delivery.
• Familiarity with Medicaid programs and the 1115 Waiver or local social services.
• Comfort using cloud-based care-management and referral platforms.
• Strong trauma-informed communication and data entry practices.

Responsibilities

• Conduct on-site outreach at food distributions, events, and partner sites to educate and enroll eligible households.
• Guide clients step-by-step through SNAP applications and gather/verify required documentation.
• Assist recipients with mandatory recertifications and prevent benefit lapses.
• Track client progress through application timelines and ensure data integrity.
• Enter and maintain accurate records in Salesforce and comply with reporting requirements.

The SNAP Nutritional Navigator provides direct, community based assistance to eligible individuals and families to facilitate enrollment in the Supplemental Nutrition Assistance Program (SNAP). This role focuses on delivering targeted outreach, conducting initial eligibility pre-screenings, and guiding clients step by step through the application and enrollment process. The Navigator ensures accurate document verification, supports clients through mandatory recertifications, and maintains consistent follow-up to resolve barriers to benefits. By offering compassionate, end to end navigation services, this position helps low income households successfully secure vital nutritional support.ResponsibilitiesCommunity OutreachConduct on-site outreach at TCAH food distributions, community events, partner sites, and other strategic community locations.Host informational sessions to educate the public about SNAP eligibility guidelines, benefits, and common misconceptions.Proactively identify and engage potentially eligible households within the community to build a robust client pipeline.SNAP Application AssistanceGuide clients step-by-step through the complete SNAP application process, ensuring a seamless and dignified experience.Help clients gather, organize, and verify required documentation prior to application submission.Submit completed applications on behalf of clients and swiftly respond to any outstanding documentation requests from social services.Recertification SupportAssist existing SNAP recipients with completing and submitting their mandatory periodic recertifications.Identify potential bottlenecks in the recertification cycle to proactively prevent any interruptions or lapses in client benefits.Client Follow-Up & TrackingFollow clients closely through every stage of the state application timeline to ensure files move forward smoothly.Track final outcomes of both initial applications and recertifications to ensure data integrity and measure program success.Data, Documentation, & CompliancePerform timely and accurate entry of all client services, interactions, and final enrollment outcomes into TCAH’s designated data systems, including Salesforce.Maintain detailed documentation and files in strict compliance with federal, state, and grant-specific reporting requirements.Performance Expectations & KPIsThe SNAP Nutritional Navigator is expected to actively contribute toward the program’s established Key Performance Indicators (KPIs). Performance will be evaluated based on the tracking and meeting of targets in the following areas:Number of SNAP pre-screenings conductedTotal applications submitted and recertifications completedVolume of unique households assistedFrequency and reach of community outreach activitiesConsistency of documented client follow-upsAccuracy and timeliness of data entry and grant documentationSupervision & Program ComplianceThe Navigator operates under the direct supervision of the Senior Benefits Access Manager and works in close coordination with the broader Benefits Access leadership team. The Navigator is responsible for executing all duties in strict alignment with the New York State Office of Temporary and Disability Assistance (OTDA) program requirements, operational performance standards, and formal reporting expectations.QualificationsEducation: Bachelor’s or Associate degree in Social Work, Human Services, Public Health, Psychology, or a related field (relevant field experience may be considered in lieu of a specific degree).Experience: 1–3 years of experience in care coordination, case management, community resource navigation, or human services delivery.System Knowledge: Direct familiarity with Medicaid programs, the Section 1115 Waiver framework, and local social services infrastructures.Technical Proficiency: Comfort operating within cloud-based care-management systems and resource referral platforms (e.g., Unite Us, Findhelp).Core Competencies: Strong trauma-informed communication skills, cross-cultural empathy, meticulous data entry habits, and the ability to manage a high-volume caseload efficiently.
