Job summary

Valle del Sol is seeking an SMI Rehabilitation Specialist to provide psychiatric rehabilitation, vocational support, and skills-based services within the Serious Mental Illness program. You will work with an interdisciplinary team to help members identify strengths, develop independent living skills, and pursue employment and education goals in the community.

Using a strengths-based, trauma-informed, recovery-oriented approach, you will develop individualized interventions promoting recovery,

Qualifications

• Experience in psychiatric rehabilitation or related behavioral health fields.
• Experience collaborating with interdisciplinary teams.
• Strong documentation and care coordination abilities.

Responsibilities

• Conduct psychiatric rehabilitation assessments and plan individualized interventions.
• Coordinate referrals to vocational rehab, education resources, and community orgs.
• Support member goals in employment, housing, education, and community integration.
• Maintain documentation and ensure quality indicators are met.
