Job summary

Interim HealthCare Inc. in Lynchburg, VA is seeking a Home Health Speech-Language Pathologist - SLP to deliver one-on-one therapy in patients' homes across Lynchburg and surrounding areas.

You will assess, treat, and monitor communication, swallowing, and cognitive goals, developing individualized plans and coordinating with a multidisciplinary team to improve daily living and quality of life.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology.
• Active SLP license in the state of Virginia.
• Minimum of 2 years of speech therapy experience (home health preferred).
• CPR certification and TB skin test required.
• Knowledge of state and federal home health regulations.
• Strong clinical judgment, critical thinking, and communication skills.
• Compassionate, dependable, and patient-focused.
• Ability to work independently in a home setting.

Responsibilities

• Provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties.
• Assess patients and develop individualized treatment plans.
• Observe and evaluate communication, swallowing, and information processing abilities.
• Monitor progress and adjust care plans as needed.
• Guide patients on mouth, breathing, swallowing, fluency, articulation, and reading exercises.
• Educate patients and families on therapy goals, safety practices, and self-care.
• Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, PT, and OT.
• Maintain accurate documentation of patient progress.

Job Title: Home Health Speech-Language Pathologist - SLP

Location: Lynchburg, VA & Surrounding Areas (In-Home Patient Care)

Schedule: Full-Time / Part-Time / PRN (Flexible Scheduling Available)

Pay: $45-$100 per visit depending on visit type + mileage reimbursement

Job Overview:

We are currently seeking a compassionate and skilled Home Health Speech-Language Pathologist - SLP to provide personalized, one-on-one therapy in a home setting throughout Lynchburg and the surrounding areas. This position is ideal for a Speech-Language Pathologist who enjoys working independently while helping patients improve communication, swallowing, and cognitive function within their daily environment.

As a Home Health Speech-Language Pathologist - SLP, you will work directly with patients recovering from illness or injury while developing individualized treatment plans focused on improving patient outcomes and quality of life.

Responsibilities of Home Health Speech-Language Pathologist - SLP:

• Provide home-based speech therapy to patients experiencing communication, swallowing, or cognitive difficulties
• Conduct patient assessments and develop individualized treatment plans
• Observe and evaluate a patient’s ability to communicate, swallow, and process information
• Monitor patient progress and modify care plans as needed
• Assist patients with mouth, breathing, swallowing, fluency, articulation, and reading exercises
• Educate patients and families on therapy goals, safety practices, and self-care techniques
• Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, PT, and OT
• Maintain accurate and timely documentation of patient progress
