Job summary

Flagstar Rehab is seeking a Speech Language Pathologist for a Skilled Nursing Facility role. You will perform speech and swallow evaluations, develop care plans, and deliver therapy to patients with diverse conditions, documenting progress in our EMR system.

The position offers flexible schedules (FT/PT/per diem), weekly pay, and multiple payment options (W-2 or 1099), with fully paid orientation and benefits options available.

Qualifications

• Current NYS SLP License and Registration.
• Clinical Competence Certification (CCC).
• CPR Certificate.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients.
• Develop plan of care and manage meal textures as appropriate.
• Provide quality therapy services to individuals in SNF settings.
• Monitor and document patient progress using EMR software.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Speech Language Pathologist Job Duties:

• Provide efficient speech or swallow evaluations and/or screens of patients
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

• Not accepting Clinical Fellows (CFY) at this time.
