Job summary

Flagstar Rehab is seeking a Speech Language Pathologist (SLP) to provide evaluations and therapy in a Skilled Nursing Facility in New York. You’ll join a team dedicated to patient-centered care and collaborate with nursing staff to support residents with communication and swallowing needs.

We offer flexible scheduling (full-time, part-time, per diem), weekly pay, and benefits including paid orientation, sick leave, commuter benefits, and 24/7 support.

Qualifications

• Current NYS SLP License and Registration.
• CCC (Clinical Competence Certification).
• CPR Certification.
• EMR software experience.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients.
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture.
• Provide quality therapy services to individuals in an SNF setting, with conditions such as TBI, CVA, Stroke, Parkinson’s, MS, Cancer, or orthopedic surgery.
• Monitor and document patient progress with the use of EMR software.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Speech Language Pathologist Job Duties

• Provide efficient speech or swallow evaluations and/or screens of patients


• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture


• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.
