Job summary

Wilmington Nursing & Rehab is seeking a Speech Language Pathologist (PRN) to join our rehabilitation team. The role includes evaluating patients, designing individualized treatment plans, and delivering direct therapy to residents in a long-term care setting.

You will document progress, set realistic goals, and educate families. Must hold a valid NY license/registration and have graduated from an accredited Speech-Language Pathology program.

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Licensed and registered to practice in NY state.
• Experience in long-term or geriatric care preferred.

Responsibilities

• Assess referred patients needing rehabilitation services.
• Provide direct patient care to assess medical condition and rehabilitation potential.
• Develop and administer treatment plans with goals based on patient capacity.
• Document patient progress and discharge documentation.
• Educate patients and families on therapy methods.
• Collaborate with disciplines to deliver patient-centered care.

Job description

Speech Language Pathologist PRN - Pay Range is $55-$62 per hour depending on years of experience.

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY
• Experience with long-term care and/or geriatric care is preferred but not limited to.
