Wilmington Nursing & Rehab is seeking a Speech Language Pathologist (PRN) to assess and treat patients in a rehabilitation setting. Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Valid state license and registration as an SLP (NY).
• Experience in long-term care or geriatric settings preferred.

Responsibilities

• Assess referred patients needing rehabilitation services.
• Provide direct patient care and evaluate medical condition and rehabilitation potential.
• Develop and administer treatment plans with goals per patient capacity.
• Set realistic goals and document patient progress and outcomes.
• Educate patients and families on therapy methods and expectations.
• Complete evaluations, progress notes, recertifications and discharges.
• Collaborate with all disciplines for patient-centered care.
• Perform duties as directed by Director of Rehabilitation.
• Maintain confidentiality per HIPAA and Residents' rights.
