Job summary

Stern Therapy Consultants is seeking a PRN Speech-Language Pathologist at Waterview Rehab and Nursing Center in Queens, NY. You will evaluate and treat adults with communication, cognitive, and swallowing needs, guided by clinical judgment and resident priorities.

Qualifications

• Active New York Speech-Language Pathologist license.
• Experience with adults with communication, cognitive, and swallowing disorders.
• Strong interdisciplinary communication and timely documentation.
• Geriatric rehabilitation experience preferred.

Responsibilities

• Complete evaluations to identify communication, cognitive-communication, and swallowing concerns.
• Develop individualized care plans aligned with residents' priorities.
• Provide therapy to improve speech, language, memory, attention, and daily decision-making.
• Manage dysphagia interventions and coordinate with nursing/dietary staff.
• Teach caregivers strategies to reinforce communication during daily care.
• Track outcomes and document progress; plan for post-discharge support.

Stern Therapy Consultants is looking for a PRN Speech-Language Pathologist at Waterview Rehab and Nursing Center. Use your clinical judgment to address the communication, cognitive, and swallowing needs of skilled nursing residents, building treatment around meaningful goals for daily living.

What You'll Do

• Complete evaluations to identify communication barriers, cognitive-communication difficulties, and swallowing concerns.
• Develop individualized plans of care that connect assessment findings with residents' priorities.
• Provide treatment supporting speech and language skills, recall of information, attention, and everyday decision-making.
• Manage dysphagia interventions and coordinate swallowing recommendations with nursing and dietary staff.
• Introduce communication supports and teach caregivers techniques they can reinforce during daily care.
• Track treatment outcomes, maintain accurate documentation, and help plan for continued support at discharge.
