Responsibilities

• Provide interventions supporting expression, understanding, memory, and problem-solving.
• Deliver dysphagia treatment and coordinate swallowing recommendations with nursing/dietary staff.
• Educate residents, families, and caregivers on practical strategies for use outside treatment sessions.
• Document evaluations and progress, adjust care plans, and participate in discharge planning.

Stern Therapy Consultants is seeking a PRN Speech-Language Pathologist at Autumn Lake at Cherry Hill. Bring your clinical expertise to a skilled nursing setting where communication, cognitive, and swallowing treatment can help residents engage in their care and navigate daily life.

What You’ll Do

• Assess speech, language, cognitive-communication, and swallowing function to identify treatment needs.
• Establish individualized goals that reflect residents' abilities, preferences, and rehabilitation priorities.
• Provide interventions that support expressing needs, understanding information, memory, and functional problem-solving.
• Deliver clinically appropriate dysphagia treatment and coordinate swallowing recommendations with nursing and dietary staff.
• Teach residents, families, and caregivers practical strategies to use outside treatment sessions.
• Document evaluations and progress, adjust care plans, and participate in interdisciplinary discharge planning.
