Job summary

Civita Care Center at Cheshire in Cheshire, CT is seeking a Speech-Language Pathologist (SLP) for a PRN opportunity to provide individualized therapy addressing communication, cognition, and swallowing for residents.

The position offers a flexible PRN schedule, a supportive rehabilitation environment, and a diverse caseload in short- and long-term care, with CT licensure and a Master’s in Speech-Language Pathology required.

Qualifications

• Active Connecticut license required.
• Master's degree in Speech-Language Pathology from an accredited program.
• Strong clinical, communication, and organizational skills.
• New graduate Speech-Language Pathologists are welcome to apply.
• Experience in skilled nursing or post-acute rehabilitation preferred, but not required.

Responsibilities

• Evaluate residents for speech, language, cognitive-communication, and swallowing needs.
• Develop individualized treatment plans based on clinical findings and resident goals.
• Provide skilled therapy addressing communication, cognition, and dysphagia.
• Monitor resident progress and adjust treatment approaches as appropriate.
• Educate residents, families, and caregivers on strategies to support communication and safe swallowing.
• Complete accurate and timely clinical documentation.
• Collaborate with nursing, rehabilitation staff, and other members of the interdisciplinary team.

We are seeking a Speech-Language Pathologist (SLP) for a PRN opportunity at Civita Care Center at Cheshire. This flexible position offers the opportunity to provide individualized therapy while helping residents improve communication, cognitive function, and swallowing abilities.

What You'll Do

• Evaluate residents for speech, language, cognitive-communication, and swallowing needs
• Develop individualized treatment plans based on clinical findings and resident goals
• Provide skilled therapy addressing communication, cognition, and dysphagia
• Monitor resident progress and adjust treatment approaches as appropriate
