Job summary

Civita Care Center at Cheshire in Cheshire, CT is seeking a Speech-Language Pathologist (SLP) for a PRN opportunity. The role focuses on evaluating residents and delivering individualized therapy to improve communication, cognition, and swallowing.

This position supports a flexible PRN schedule and a collaborative rehabilitation team. Active CT SLP license, a Master’s degree in Speech-Language Pathology, and strong clinical skills are required; new graduates are welcome.

Qualifications

• Active Connecticut Speech-Language Pathologist license is required.
• Master's degree in Speech-Language Pathology from an accredited program.
• Strong clinical, communication, and organizational skills.
• Skilled nursing or post-acute rehabilitation experience is preferred.
• New graduates are welcome to apply.

Responsibilities

• Evaluate residents for speech, language, cognitive-communication, and swallowing needs.
• Develop individualized treatment plans based on clinical findings and resident goals.
• Provide skilled therapy addressing communication, cognition, and dysphagia.
• Monitor resident progress and adjust treatment approaches as appropriate.
• Educate residents, families, and caregivers on strategies to support communication and safe swallowing.
• Complete accurate and timely clinical documentation.
• Collaborate with nursing, rehabilitation staff, and other members of the interdisciplinary team.
