Job summary

Sunnyvale Post Acute in Sunnyvale, CA is seeking a Speech-Language Pathologist (SLP) to evaluate and treat residents with communication, cognitive, and swallowing needs. We offer Full-Time, Part-Time, and PRN opportunities with pay rates of $60.00-$65.00 per hour, DOE, plus comprehensive benefits for eligible employees.

Qualifications

• Current California Speech-Language Pathologist (SLP) license.
• Appropriate certification and credentials required to practice in California.
• Experience in skilled nursing, post-acute care, or rehabilitation preferred.
• Strong communication, clinical, and organizational skills.
• Compassionate, resident-centered approach.
• Ability to work effectively as part of an interdisciplinary team.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate residents and develop individualized speech, language, cognitive, and swallowing treatment plans.
• Provide skilled therapy services based on resident needs and established care plans.
• Monitor resident progress and modify treatment approaches as appropriate.
• Complete accurate and timely clinical documentation.
• Collaborate with nursing, therapy, dietary, and other interdisciplinary team members.
• Educate residents, families, and caregivers regarding treatment recommendations and strategies.
• Promote resident safety, independence, communication, and quality of life.
• Maintain compliance with applicable professional, state, and federal standards.
