Job summary

Evolve Therapy Services is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Columbus, NE. This full-time role focuses on planning, organizing, developing, and directing speech language pathology services to ensure the highest quality resident care.

You will evaluate patients, create treatment plans with short- and long-term goals, educate patients, families, and staff, and ensure CMS-compliant documentation in NetHealth EMR.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency (CCC) required.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instructs patient, family, and staff to carry over therapeutic remediation and goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversight and implementation of FEES/MBS protocols with site coordination.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency (CCC)

Active SLP license in state(s) of practice
