Job summary

Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility located in Columbus, Nebraska. The role focuses on planning, directing speech pathology services to ensure high-quality resident care, with emphasis on compliance with CMS and regulatory standards.

You will carry out therapy services, develop treatment plans with goals, educate patients and staff, document in NetHealth EMR, and participate in FEES/MBS protocols.

Qualifications

• Master's degree in Speech Language Pathology is required.
• CCC-SLP certification is required.
• Active SLP license in applicable states.
• Commitment to ongoing continuing education.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and short-term, and long-term goals.
• Instruct patients, families, and staff in speech-language procedures.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to identify residents' needs timely.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
