Job summary

Evolve Therapy Services, LLC is seeking a part-time Speech Language Pathologist for an in-house Skilled Nursing Facility in Omaha, NE. The role focuses on planning, directing, and delivering speech-language pathology services while ensuring compliance with CMS and licensure requirements.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in states of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target list, short-term, and long-term goals.
• Instruct patient, family, and staff to facilitate carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to meet resident needs.
