Job summary

Evolve Therapy Services in Columbus, NE seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility to provide clinical services and ensure high-quality resident care.

You will evaluate data, develop treatment plans with short- and long-term goals, instruct patients and families, complete CMS-compliant documentation in NetHealth EMR, oversee FEES/MBS protocols, and coordinate with site staff to meet resident needs.

Qualifications

• Master’s degree in Speech Language Pathology required.
• CCC (Certificate of Clinical Competency) required.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide high-quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists, short-term and long-term goals.
• Instruct patients, families, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.
