Job summary

Evolve Therapy Services, LLC in Grand Island, NE is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis.

You will plan, organize, and direct SLP services, evaluate data for treatment, develop goals, and document in CMS-compliant NetHealth EMR. Requirements include a Master’s degree in Speech Language Pathology, CCC, and an active license in state(s) of practice.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Must have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards and productivity.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instruct patient, family, and staff to ensure carryover of remediation goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee protocols for FEES/MBS and coordinate site programs.
• Participate in resident screening to ensure needs are met timely and with clinical excellence.
