Job summary

Somers Manor Rehabilitation & Nursing Center seeks a Speech Language Pathologist to join the rehabilitation team. The SLP will assess patients, develop treatment plans, and collaborate with the care team to optimize outcomes in a long-term care setting.

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Licensed and registered to practice in NY (as indicated).
• Experience in long-term care or geriatric settings preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and plan direct patient care.
• Develop and administer a treatment program with goals based on capacity and tolerance.
• Document patient condition; record progress notes, evaluations, and discharge planning.
• Educate patients and families on therapy methods and care plans.
