Job summary

Somers Manor Rehabilitation & Nursing Center is seeking a dedicated Speech Language Pathologist to join its rehabilitation team. The role involves direct patient care, assessment, and developing individualized therapy plans for residents in a long-term care setting.

Applicants must have graduated from an accredited Speech Language Pathology program and hold a valid NY licensure. Experience in geriatric care and skilled nursing is preferred. Competitive salary and benefits are offered.

Speech Language Pathologist - SLP

Somers Manor Rehabilitation & Nursing Center is looking for a talented and hard-working Speech Language Pathologistto join their ever-growing rehabilitation team.

Somers Manor Rehabilitation and Nursing Center, a family owned Rehabilitation and Skilled Nursing Facility for over 55 years, is nestled on more than 100 acres in Northern Westchester County, New York. The Rehabilitation Care is rendered with great clinical acumen and advanced technology. The warm and home-like atmosphere contributes to the quality of life of the Long Term Care Community.

Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
