Job summary

MCMCFC is seeking a Skin Integrity Coordinator, RN to develop, implement, and evaluate the skin integrity program across the facility. The role requires applying nursing expertise to prevent pressure injuries, monitor outcomes, and coordinate care with rehab, nutrition, home health, and other teams.

Qualified applicants must hold an active Michigan RN license, have at least four years of nursing experience, and possess BLS. Wound care credentials are preferred.

Qualifications

• Be licensed by the Michigan Board of Nursing to practice as RN and maintain licensure.
• Minimum four (4) years as a nurse in a healthcare setting.
• BLS certification required before start date.
• One year prior wound care experience required.
• Ability to plan, organize, develop, implement and interpret programs and policies.
• Ability to exercise professional judgment for resident welfare and safety.

Responsibilities

• Participates in training and orientation of new employees with emphasis on skin care and prevention of pressure ulcers.
• Monitors and documents cost effectiveness of treatment provided.
• Establishes staff goals and directives including performance improvement processes.
• Provides consultation and direct social assistance in managing patients with altered skin integrity.
• Coordinates resident care with rehab, nutrition, diabetic education, home health, DME and other services.
• Evaluates and reports on QAPI achievement for outcomes.
• Works with Staff Development to provide in-service training on wound care and skin care.
• Collects, analyzes and documents wound-related information and statistics.
• Participates in admission process including skin integrity assessments and care plans.
• Discloses resident information in accordance with HIPAA and reports violations.
• Coordinates with interdisciplinary team to plan services during admission and discharge.
• Documents interactions regarding resident progress and care plans.
• Participates in Medicare and managed care meetings.
• Assists in discharge planning and related services.
• Performs random charting audits to support reimbursement.
• Engages residents and families; reports issues to department heads.
