Job summary

FutureCare is seeking a Wound Care Coordinator to provide skin care for residents, focusing on prevention and treatment of skin disorders. You will be part of our frontline care team and play a pivotal role in planning, implementing, and evaluating skin care programs across the facility.

The role requires an active Maryland RN license with wound care certification, and prior SNF experience is preferred. You will oversee wound care activities, conduct weekly skin rounds, and educate residents and

Qualifications

• Must hold a current RN license in Maryland.
• Must have an active certification in wound care.
• At least one year experience in a skilled nursing facility preferred.
• Previous experience in skin care treatment is a plus.
• Ability to work independently and accurately with required documentation.
• Strong communication skills to facilitate interactions with residents and staff.

Responsibilities

• Identify, manage, and treat specific skin disorders.
• Oversee all direct wound care activities to ensure practices are consistent.
• Coordinate weekly skin rounds.
• Perform assessment evaluations using observation and inspection.
• Collaborate with Care Planning Team on treatment development within wound care protocol.
• Monitor prevention measures such as inspection, turning and positioning.
• Modify treatment regimens to resident needs within medical, regulatory, and facility policies.
• Provide information to residents and caregivers on prevention techniques and overall health.
