Job summary

Rockport Healthcare Services is seeking a Treatment Nurse (RN or LVN) to oversee prescribed treatments for facility residents under the direction of the Director of Nursing Services. The role emphasizes wound assessment, documentation, timely treatment initiation, and coordination with the care team.

The position requires a nursing license in the state, at least one year in a skilled nursing setting, and solid English communication.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current licensure as a Registered Nurse or a Licensed Vocational Nurse in the state in which practicing.
• 1+ year experience in a skilled nursing facility.
• Ability to read, write and speak the English language.
• Possess mental and physical health.

Responsibilities

• Assess resident’s skin condition on admission, re-admission, and after leaves of absence.
• Initiate Skin and Wound Progress reports and notify physicians of skin issues.
• Ensure treatment orders exist for all skin problems and document on care plans.
• Photograph residents with skin issues and stage 2–4 ulcers at admission and discharge.
• Document findings in the Plan of Care and weekly Nurse’s Skin Wound Progress reports.
• Coordinate with Dietitian, Physical Therapist, and other staff for wound management and care.
• Audit treatment sheets and monitor monthly charges for related supplies.
• Take cultures as ordered and attend in-service trainings.
