Job summary

Cantex Continuing Care Network is seeking a Nurse Manager to oversee nursing care at Mill Creek - Silsbee, TX. This leadership role focuses on ensuring high-quality patient care under physician prescriptions and established standards.

The Nurse Manager will supervise care delivery, monitor records, and coordinate interdisciplinary team activities to maintain optimal patient outcomes. The position requires TX licensure and prior supervisory experience in long-term care.

Qualifications

• Graduation from an accredited Nursing School and a current and valid LVN/LPN license in Texas.
• Minimum of one-year supervisory experience in a long-term or health care setting is preferred.
• Ability to read, write, and speak the English language.

Responsibilities

• Monitor and review daily records for accuracy, documentation and intervention.
• Make rounds on all patients at least every 4 hours and communicate findings to the charge nurse.
• Review the 24-hour report book for changes in patient condition and coordinate interventions.
• Review incident reports and ensure proper nursing assessment and documentation.
• Review discharged patient records for accuracy and ensure post-discharge plans are completed.
• Distribute the monthly MDS/care plan calendar and complete required assessments and plans.
