Job summary

Cantex in Silsbee, TX is seeking an experienced Nurse Manager to participate in and oversee patient care as prescribed by the attending physician, in accordance with accepted standards of practice.

The role requires LVN/LPN licensure, supervisory experience, and English proficiency; you will lead rounds, review records, ensure documentation, and coordinate with the Interdisciplinary team to maintain quality care.

Qualifications

• Graduation from an accredited Nursing School and a current LVN/LPN license in the applicable State.
• Minimum of one-year supervisory experience in a long-term or health care setting is preferred.
• Ability to read, write, and speak the English language.

Responsibilities

• Make rounds on all patients at a minimum of every 4 hours and as needed; communicate findings to the charge nurse.
• Review 24-hour report books and assess patients with recent changes in condition.
• Review incident reports and ensure proper nursing assessment and notification.
• Review medical records on discharge and follow up with post-discharge plans of care.
• Ensure documentation on patient care events (admissions, transfers, follow-ups).
• Distribute monthly MDS/care plan calendar and complete MDS assessments and TILE assessments.
• Attend weekly care plan meetings and interdisciplinary team meetings.
• Enter physician orders daily and ensure accuracy of orders in patient records.
• Complete recommendations from consultants within specified timeframes.
