Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapist for an in-house Skilled Nursing Facility partner located in Oklahoma City, OK. This full-time position includes premium pay and relocation bonus options.

You will plan, develop, and direct physical therapy services, perform evaluations, create treatment plans, and document in NetHealth EMR, coordinating with residents, families, and the interdisciplinary team to ensure high-quality resident care.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound clinical judgment in evaluation, planning, implementation and follow-up of therapy programs.
• Conduct regular screenings to determine need for intervention/treatment.
• Screen residents referred to physical therapy to determine the need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
