Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Oklahoma City, OK. This full-time position offers premium pay and a relocation bonus. The role requires a BS in Physical Therapy and an active PT license.

Responsibilities include planning and directing therapy services, adhering to CMS and company documentation standards, and communicating progress to residents and the interdisciplinary team. CE credits are expected to maintain licensure.

Qualifications

• Bachelor of Science Degree in Physical Therapy (accredited program).
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop and implement treatment plans with short- and long-term goals.
• Conduct screenings to determine need for intervention and treatment.
• Communicate findings and recommendations to residents, families, and the interdisciplinary team.
• Ensure compliant and medically necessary documentation in NetHealth EMR.
