Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Oklahoma City, OK. This full-time position offers premium pay and relocation bonus.

You will provide PT services, document in CMS-compliant NetHealth EMR, evaluate residents, develop treatment plans, and communicate findings to residents and the inter-disciplinary team. Must hold BS in Physical Therapy and an active license.

Qualifications

• Bachelor of Science Degree in Physical Therapy required.
• Active Physical Therapy license in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill services per CMS requirements and NetHealth EMR.
• Develop treatment plans with short and long-term goals and communicate findings to residents and interdisciplinary team.
