Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill PT services per CMS and company procedures.
• Evaluate and develop resident therapy programs.
• Screen residents regularly to assess intervention needs.
• Develop treatment plans with short and long-term goals.
• Communicate findings to residents, families, and care team.
• Ensure CMS-compliant documentation in NetHealth EMR.
