Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Oklahoma City, OK. You will plan, implement, and direct physical therapy services to ensure high-quality resident care in line with federal, state, and local standards.

Responsibilities include assessments, developing treatment plans, documentation, and collaboration with residents, families, and the interdisciplinary team. This is a full-time role with premium pay and relocation bonus options.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assume responsibility for ongoing professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of therapy programs.
• Conduct regular screenings to determine need for intervention.
• Conduct timely screenings of residents referred to physical therapy to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
