Job summary

TeamCare Medical in New York seeks a Site Physician to lead medical care at the CLHC/Day Treatment Center under the Chief Medical Officer. You will oversee clinical and administrative aspects and collaborate with an interdisciplinary care team.

This role requires board certification in Internal Medicine or Family Practice, residency in these fields, and experience with geriatric care. Strong communication skills and a commitment to quality care are essential.

Qualifications

• Residency in Internal Medicine or Family Practice is required.
• Fellowship, and diploma or experience in geriatrics and palliative care is preferred.
• Board certification in Internal Medicine or Family Practice is required.
• No malpractice or loss of license at the DTC.
• Excellent verbal and written communication skills.
• Minimum of one year of experience working with a frail or elderly population or must receive training if less than one year.

Responsibilities

• Direct the medical care and treatment of patients with primary medical responsibility over the care team.
• Collaborates with site NP and community-based primary care providers.
• Identifies opportunities to improve quality, cost, and utilization of care for the site participants.
• Carries a caseload of patients as PCP and participates in direct medical care.
• Leads the medical presence during start of care events.
• Participates in multidisciplinary team activities and case conferences.
