Job summary

TeamCare Medical in Brooklyn, NY is seeking a Site Physician to oversee medical care at our Day Treatment Center. Under the direction of the Chief Medical Officer, you will lead a multidisciplinary team, supervise clinical care, and ensure high-quality patient outcomes across the DTC setting.

The role requires Board Certification in Internal Medicine or Family Practice, plus geriatric and palliative care experience.

Qualifications

• Graduate of an accredited school of medicine.
• Board Certification in Internal Medicine or Family Practice.
• Board Certification or experience in geriatrics and palliative care.
• No malpractice or loss of license at the DTC.
• Excellent verbal and written communication skills.
• Minimum of one year of experience working with a frail or elderly population or receive training if less.

Responsibilities

• Direct medical care and primary medical responsibility for the care team.
• Collaborates with site NP and community-based PCPs on shared population.
• Identify opportunities to improve quality, cost, and utilization of care.
• Participate in HR-HI panel discussions to improve participant health.
• Carry a caseload of patients as PCP and participate in direct medical care.
• Lead medical presence at start of care events.
• Participate in multidisciplinary team activities and case conferences.
• Develop and supervise ambulatory medical services.
• Facilitate relations with subspecialists and other institutions.
• Develop preventive health screening and outreach programs.
