Job summary

CenterLight Health System seeks a Site Physician to oversee medical care at the Day Treatment Center in Brooklyn/ Bronx, Mon-Fri 8:00 AM–5:00 PM, 40 hours weekly. You will direct clinical care, supervise staff, and coordinate with NP and other providers to improve quality, utilization, and patient engagement.

The position requires MD, NYS license, board certification in Internal Medicine or Family Practice, and geriatric/palliative experience preferred.

Qualifications

• MD from an accredited medical school
• Board certification in IM or FP required
• Geriatrics/palliative care fellowship or experience preferred
• Able to obtain NYS license and Medicaid/Medicare IDs

Responsibilities

• Direct the medical care and treatment of patients with primary medical responsibility over the care rendered by the interdisciplinary team
• Collaborate with site NP and community-based providers on shared patient populations
• Identify opportunities to improve quality, cost, and utilization of care for site participants
• Lead medical presence during start-of-care events and participate in IDT meetings
• Develop and supervise ambulatory medical services and preventive health programs
• Serve as liaison with external medical institutions and subspecialists
• Facilitate medical education for staff, trainees, and students
• Conduct patient assessments and provide care in DTC and patients' homes
• Participate in Utilization Review and quality assurance activities
