Qualifications

Bachelor's degree in Physical Therapy from an accredited program.
Licensed Physical Therapist (PT) with current credentials.
Basic Life Support (BLS) certification within 30 days of hire.
Commitment to ongoing continuing education in acute stroke care.

Responsibilities

Demonstrates ability to perform patient transfers with assistive devices and safe body mechanics.
Delivers patient care with dignity, empathy and engagement for patients and families.
Designs and updates a care plan aligned with goals, treatment, outcomes and discharge planning.
Educates patients, families and staff using effective teaching methods.
Evaluates clinical findings to determine PT diagnoses and outcomes of care.
Performs examinations using reliable methods relevant to chief complaint and history.
Documents in the EMR per policy CTSCP67.
Screens patients to determine need for therapy services and effectiveness of care.
Oversees support personnel, providing regular feedback and ensuring compliance with standards.
