Job summary

AccentCare, Inc. is seeking an RN Case Manager for Home Health to manage patient care plans from admission through discharge. The role leads RNs, LPNs, and CHHAs to ensure quality care, coordinating interdisciplinary needs and care delivery in Burnsville and surrounding areas.

The position offers a $5,000 sign-on bonus, a Monday–Friday schedule, and pay in the range of $87,000 to $102,000 annually plus per-point compensation. Licensed RN in Minnesota and CPR certification required.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in Minnesota

Responsibilities

• Assessment of nursing needs and overall management of the patient's care plan
• Initial patient assessment upon admission including patient history
• Selects nursing diagnoses and assesses medicines for adverse reactions
• Establishes goals aligned with diagnosis and informs physician
• Implements nursing interventions and re-evaluates patient needs
• Documents assessments, progress, and discharge notes timely
• Informs physicians of changes in patient condition and needs
