Job summary

AccentCare, Inc. in Minnesota is seeking an RN Case Manager for Home Health. This full-time role services Burnsville and surrounding areas with a Mon–Fri, 8:00 a.m.–5:00 p.m.

schedule and an annual pay of $87,000–$97,000, plus a $5,000 sign-on bonus. Compensation is paid on a per-point basis. The RN Case Manager will oversee care plans from admission to discharge, coordinating staff and interdisciplinary clinicians to ensure quality patient care.

Qualifications

• RN license in the state of operation.
• Minimum of one year RN experience.
• CPR certification required.

Responsibilities

• Assess nursing needs and manage the patient's care plan.
• Complete initial admission assessment with patient history.
• Select appropriate nursing diagnoses.
• Review medicines for adverse reactions and allergies.
• Set realistic, measurable goals for care.
• Communicate assessment results and interventions to physician.
• Implement nursing interventions per diagnosis and goals.
• Re-evaluate patient needs as required.
• Document assessments, progress, and discharge notes timely.
• Inform physician of changes in patient condition.
