Job summary

Columbus House, Inc. in New Haven, CT, seeks a Social Innovations Fund (SIF) Case Manager to provide housing case management for clients seeking housing and services. The role focuses on housing applications, searches, and independent living toward self-sufficiency, with a 32-hour full-time schedule.

Responsibilities include outreach, collaboration with community providers, and maintaining client records. Bi-lingual English/Spanish is preferred; valid driver's license required; bachelor s degree

Qualifications

• Bachelors degree required; writing skills essential.
• Experience in case management or housing services preferred.
• Bi-lingual English/Spanish preferred.

Responsibilities

• Provide housing case management for SIF clients in the New Haven region.
• Assist with housing applications, housing search and independent living goals.
• Coordinate with mental health, substance abuse and other health care providers as needed.
• Maintain client records in paper and electronic formats per agency standards.
• Interact with landlords on behalf of clients and manage a caseload of ~20 individuals.
