Job summary

Columbus House, Inc. in New Haven, CT, is seeking a PSH Case Manager for the Social Innovations Fund (SIF) program. The role focuses on housing case management, assisting clients with housing applications and searches, and attaining self-sufficiency.

The position requires a Bachelor’s degree, experience in case management or supportive housing, and a valid driver’s license. Salary ranges from $25.50 to $27.50 hourly with full-time benefits.

Qualifications

• Experience in case management, supportive housing or related field.
• Knowledge of Fair Housing Laws; sound judgment and decision making; and solid math skills.
• Strong organizational skills and attention to detail; motivated to help clients succeed.
• Excellent writing skills and ability to communicate clearly.

Responsibilities

• Assist and monitor SIF clients from entry into the program until discharge in collaboration with community providers.
• Outreach and engage clients; provide case management services.
• Apply for state housing assistance and assist with securing affordable housing; conduct regular check-ins with clients.
• Maintain connections with mental health, substance abuse and other health care providers.
• Maintain client records (paper and electronic)
• Interact with landlords on behalf of clients as needed.
• Manage a client caseload of ~20 individuals.
• Other related duties as assigned.
