Job summary
Northwestern Medicine is seeking a Nurse Practitioner or Physician Assistant to join our Sickle Cell Disease program, focusing on inpatient consults with a smaller outpatient component. The APP will coordinate care with hematology, hospital medicine, and care coordination teams to improve continuity and outcomes for a high-acuity patient population.

Qualifications
- Master’s degree from an accredited school/college of nursing OR master's degree from an accredited Physician Assistant Program.
- Acute care certification required for APRNs.
- Current Illinois RN license and APRN license or PA license with privileges.
- CPR/BLS certification.
Responsibilities
- Conducts comprehensive appraisals of patients' health status and performs physical examinations.
- Provides direct clinical care and prescribes medications and therapies.
- Orders and interprets laboratory tests and imaging as clinically appropriate.
- Develops and coordinates patient education and discharge planning.
- Collaborates with multidisciplinary teams to improve care pathways and outcomes.
