Job summary

Sweetwater Care in Fresno, CA is seeking a licensed practical nurse (LPN) to deliver patient care in a long-term care setting. You will administer medications, perform treatments, and educate patients and families while following physician orders and center policies.

Role requires LTC experience or willingness to learn; familiarity with PointClickCare or paper charting is a plus. Collaboration with the medical team and on-site duties are included.

Qualifications

• Currently licensed as LPN in state.
• Long-term care experience preferred but not required.
• Experience with long-term care settings and patient care.
• PointClickCare charting experience or willingness to use paper charting.

Responsibilities

• Provides nursing care within scope of practice and adheres to standards and physician orders.
• Demonstrates knowledge and uses infection control guidelines per manual.
• Notifies departments of patient changes and participates in discharge/planning processes.
• Educates patients and families about care plans and treatments.
• Administers medications and treatments timely according to facility policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient's needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
• One-year prior nursing management experience preferred, not required.
