Job summary

Sweetwater Care in California is seeking an LVN to provide nursing care within the scope of practice, following physician orders and center policies. The role supports quality improvement and patient education while ensuring timely medication administration and treatments.

The ideal candidate will hold a California LVN license (compact accepted) with experience in Long Term Care and charting systems, and will participate in rounds with physicians as needed.

Qualifications

• Licensed LVN in California (or valid compact license).
• Experience in nursing management preferred.
• Experience in Long Term Care and charting systems.

Responsibilities

• Provide nursing care per scope of practice and policies.
• Apply infection control practices per manual.
• Participate in Quality Improvement initiatives.
• Educate patients and families about care plans.
• Administer medications and treatments per policy.
• Transcribe and carry out physician orders as allowed.
• Accompany physicians on rounds when needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
• Long Term Care experience
• Paper charting and point click care charting experience
