Job summary

Community Bridges, Inc. (CBI) seeks a Client Care Coordinator to manage housing caseloads and coordinate care for individuals/families experiencing homelessness.

You will connect clients to PCMH/PCP services and monitor community-based services across the continuum of care. The role requires coordinating with RBHA and non-RBHA providers, documenting outcomes, and supporting clients with complex housing, health, and behavioral health needs.

Qualifications

• Highschool diploma or GED required; associate degree in Behavioral Health preferred.
• Minimum 1 year recovery from substance use and/or mental health disorders.
• Minimum 6 months paid related work experience in behavioral/medical crisis settings.
• 1–3 years full-time healthcare-related work experience preferred.
• Complete CBI Peer Certification within 90 days of hire.
• Arizona Fingerprint Clearance card required.

Responsibilities

• Coordinate care for shelter residents and link to PCMH/PCP services.
• Monitor RBHA and non-RBHA services and document outcomes.
• Prepare end-of-shift reports detailing coordination of care.
• Collaborate with multi-disciplinary team within the CBI continuum of care.
