Job summary

Clinical & Support Options, Inc. is seeking a Shelter Community Support Provider to work with shelter guests in Greenfield, MA.

This role delivers intensive clinical case management, helps with daily living skills, and supports access to mental health services in shelter or outreach settings. You will build a natural support network, assist with housing upkeep, and coordinate activities including employment or volunteering, while maintaining strong ties with families and therapists for member

Qualifications

• Bachelor's or Associate's degree in a mental health discipline or high school diploma with 5 years related experience.
• Valid driver's license and reliable transportation.
• Bilingual candidates are strongly encouraged to apply.

Responsibilities

• Provide intensive, clinical case management to consumers with serious mental illness in shelter or outreach settings.
• Offer skills training in daily living activities and assistance with transportation, money management, cleaning, laundry, cooking, grooming and health care.
• Assist with advocacy needs and facilitate mediation as appropriate.
• Help identify and obtain entitlements to meet consumer needs.
• Provide counseling and intensive support during psychiatric crises or transitions.
• Assist with identifying mental health and medical services and accessing services.
• Develop and maintain a natural support network of family and friends.
• Maintain apartment/shelter space upkeep for healthful, safe living.
• Coordinate meaningful day activities, including employment or volunteer opportunities.
• Arrange transportation for consumers following program guidelines.
• Maintain contacts with family, natural helpers, outpatient therapists and others to support consumers.
