Job summary

Clinical & Support Options, Inc. (CSO) is seeking a Shelter Community Support Provider to work with shelter guests in Northampton. The role includes intensive, clinical case management and outreach support, with services provided at the shelter or in outreach settings.

Qualifications

• Bachelor's or Associate's degree in a mental health discipline or high school diploma with five years' related experience.
• Valid driver's license and reliable transportation.
• Bilingual candidates are strongly encouraged to apply.

Responsibilities

• Provide skills training and assistance in daily living activities (transportation, money management, cleaning, laundry, food prep, grooming, health care).
• Assist with advocacy needs; facilitate mediation as appropriate.
• Identify and help obtain entitlements meeting the consumer's needs.
• Provide counseling and intensive support during psychiatric crises or transitions.
• Help consumers identify appropriate mental health and medical services and access services of choice.
• Assist in developing and maintaining a natural support network of family and friends.
• Maintain upkeep of apartment or shelter space for healthful living.
• Assist with negotiating relationships with others.
• Coordinate meaningful day activities, including employment or volunteer work.
• Arrange transportation for consumers as appropriate per program guidelines.
• Maintain contacts with family, natural supports, therapists, and others to support consumers.
