Job summary

Shepherd’s House Ministries in Central Oregon is seeking a part-time Client Care Coordinator to support shelter residents by navigating healthcare, coordinating services, and providing trauma-informed, person-centered assistance under the RN Supervisor.

The role emphasizes relationship-building, documentation, and collaboration with nurses, case managers, and community partners to help residents set and reach health goals while maintaining professional boundaries.

Qualifications

• High school diploma or equivalent required.
• Completion of a recognized Peer Support training program, or ability to complete required training within the specified timeframe.
• Experience working with individuals experiencing homelessness, poverty, healthcare access barriers, behavioral health challenges, substance use, or similar barriers.

Responsibilities

• Build trusting, respectful relationships with shelter residents through consistent, trauma-informed, person-centered engagement.
• Help residents identify healthcare needs and goals and connect with appropriate medical, behavioral health, substance use, dental, vision, and other supportive services.
• Support residents with scheduling, preparing for, attending, and following up on healthcare appointments, hospital visits, referrals, and recommended services.
• Help residents navigate healthcare systems and overcome barriers related to insurance, benefits, transportation, medical records, identification, pharmacy access, and other basic needs.
• Coordinate communication and services among residents, shelter staff, healthcare providers, and community partners while maintaining appropriate confidentiality.
• Help residents understand healthcare information, prepare questions for providers, and reinforce health and wellness education provided by licensed professionals.
• Identify and promptly communicate observable health, safety, medication, behavioral health, or other concerns to the RN Supervisor or appropriate healthcare professional.
• Connect residents with preventive care, behavioral health, substance use treatment, harm-reduction, overdose-prevention, and other community resources as appropriate.
• Maintain accurate and timely documentation of resident contacts, referrals, services, follow-up activities, and applicable program outcomes.
• Collaborate with nurses, case managers, peer support staff, shelter staff, healthcare providers, and community partners while maintaining professional boundaries and working within the role’s established non-clinical scope.
