Job summary

Paycom - ATS is seeking a dedicated case manager to provide structured, one-on-one support at an emergency shelter in Virginia. The role involves intake, needs assessments, and developing individualized recovery plans while coordinating with external professionals to meet residents’ holistic needs.

The position requires an associate degree in a related field and at least one year of direct case management experience. It offers a stable schedule and benefits such as PTO and retirement options.

Qualifications

• Associate degree in social work, human services, or related field from an accredited college or university; AND
• One year experience providing direct case management social services including assessing residents’ needs and developing long-term recovery plans; OR
• Equivalent combination of training and experience meeting requirements.

Responsibilities

• Conducts resident intake and initial needs assessment at shelter entry.
• Provides linens, toiletries, and hygiene products; replenishes as needed.
• Meets with residents to discuss progress, feelings, and growth; documents progress.
• Prepares and maintains case records and logs; enters information into HCIS.
• Refers residents to additional counseling resources as needed.
• Maintains awareness of progress with external case management professionals.
