Job summary

Project Renewal seeks a dedicated Case Manager for East Williamsburg Men’s Shelter in New York City. You will oversee assessments, counseling, and comprehensive case management for a 25-35 client caseload, enabling clients to pursue independent housing and employment.

Under the Director of Social Services, you’ll implement living plans, coordinate with service providers, and support residents through group and individual sessions in a supportive shelter setting.

Qualifications

• High School/GED or Associate's Degree required.
• 1-2 years experience with homeless adults, substance use or mental health populations.
• Teamwork, organizational, interpersonal, communication, and computer skills; bilingual a plus.

Responsibilities

• Manage a caseload of 25-35 clients.
• Conduct intake and psychosocial assessments.
• Develop and monitor Individual Living Plans with objectives.
• Input client data into CARES system.
• Counsel clients in individual and group sessions.
• Coordinate with community organizations and housing providers.
• Organize group activities and educate clients.
• Maintain written documentation per guidelines.
• Provide escorts to meetings and appointments.
• Perform other duties as assigned.
