Job summary

Living Innovations is seeking a Community Living Coordinator in Millinocket, ME to support adults with IDD in Shared Living arrangements. You’ll be the primary liaison ensuring safe, supportive, home-like environments and coordinating care with DSPs and families.

Qualifications

• High school diploma or GED required.
• Bachelor's degree preferred.
• One year of IDD or related healthcare experience required.
• Valid driver’s license and reliable transportation.
• Ability to pass comprehensive background checks per state and company regulations/policies

Responsibilities

• Conduct bi-monthly in-person home visits to provide support and ensure the quality of shared living services.
• Oversee and manage Direct Support Professionals (DSPs).
• Collaborate with individuals with IDD, their families, and other professionals to develop individualized support plans.
• Advocate for individuals' needs and connect them with community resources.
• Motivate and support your DSP team to deliver exceptional care.
• Maintain accurate documentation and ensure regulatory compliance.
