Job summary

Integrated Life Choices is seeking a Shared Living Provider Coordinator to oversee contract holders of shared living providers and to develop, implement, and train habilitation programs for individuals served in SLP homes.

The role represents a liaison between the state, ILC, and SLPs, coordinating onboarding, ongoing supervision, and regular program assessments to ensure compliance and high-quality supports.

Qualifications

• Ability to read, analyze, and interpret general business materials.
• Ability to write reports and correspondence.
• Ability to present information clearly to groups of managers and guardians.
• Ability to foster relationships with families and organizations.

Responsibilities

• Communicates with service coordination, SLPs, guardians, and medical professionals.
• Conducts onboarding and interviews with potential SLPs.
• Performs monthly home visits to ensure compliance and expectations.
• Manages billing, attendance, and program updates.
• Ensures consent forms and rights/due process forms are current.
• Keeps ISP information up to date and attends ISP meetings.
