Job summary

Moms In Motion is seeking a Service Facilitator (SF) Case Manager in Virginia to visit clients in their homes and guide families through Medicaid Waiver programs. You’ll be the trusted resource for care plans, assessments, and coordinating services from a home-base role.

The role emphasizes compassionate service, organizational efficiency, and strong communication, with travel to client homes and flexibility to work remotely from a laptop/tablet.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required (we care more about heart and experience).
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license & reliable vehicle (no client transport).
• Great communication—both written and spoken.
• Able to pass a background check + provide 2 professional references.
• Bonus points if you’ve got Person-Centered Thinking/Planning training.

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
